Billing Policies
Insurance Card – At each appointment, please bring your insurance card.
Cancellation – If you must cancel an appointment, kindly give us 24 hours’ notice. We regret having to charge for missed appointments and cancellations with insufficient notice. We do this reluctantly, but in consideration of the time lost for both our patients and our doctors.
Referrals: If your insurance company requires one, get one from your primary care physician (PCP) well before your appointment if you need to see a specialist. To process a referral, some PCP offices require up to 72 hours’ notice. The referral is your responsibility. If your insurance company requires a referral, our doctors won’t see you without one.
Co-Pays are due at the time of your appointment. You may pay by cash, check or credit card. We also accept Mastercard, Visa.
