How do I request my medical records from CareNow® Urgent Care?
You must complete a Medical Release Authorization form if you want a copy of your medical records. The release form needs to be fully filled out in order for you to get a copy of your records. The release of your records may be delayed if the form is incomplete. The Medical Records Department can be contacted by email or FAX at (855) 874-5286 with the completed form and a copy of your ID.
